Type your petition, attach any necessary supporting documents, and, from their Rice email address, email it to firstname.lastname@example.org before the committee's weekly meeting. Please be sure to include your full legal name, student ID number, college, and a number you can be reached.
If you have a confirming statement from your professor, it must be written on department letterhead or emailed directly to email@example.com . If you missed the deadline because of a medical emergency, or family crisis, you will need to submit supporting documentation.
